• If you have received the patient registration form, please complete and return it to us as soon as possible.  Be sure to include a copy of the front and back of your insurance card(s) and a copy of your photo ID.

• Please have a list of all the medication you take (prescription and over the counter).

• Be prepared to discuss any medical conditions you may have and your medical history.

• Let us know if you have medication allergies.
